Nigel,
What happens to me is that if I have Outlook 98 or 2000 (Probably the same with 97) with two services installed - Internet E-mail and MS Exchange, when you do a send() with Mapi, it runs a full send and receive. That means that first it goes to the internet e-mail service and does a send and receive, after it goes to MS Exchange and does a send and receive.
In MS Exchange, you generaly have it configured to receive messages as they are delivered by the server so you don't notice the difference. But with internet e-mail you can see it sending and receiving new mail.
There is no way to avoid the two operations as the recipient you are sending mail to can be in the local MS Exchange or in Internet-Mail (Outlook resolves the names).
But it would be good if there was a way to just send and not receiving mail.
