I figured out how to use this by using Microsoft's Help Builder to put together a tiny application and then reverse-engineering the TOC and Index that they built. Pretty easy that way.

I did this for my engine that produces HTML reference docs out of Fox projects, which I'll be uploading Real Soon Now.
