Harry,
When worse comes to worse and I've exhausted all my logical and practical problem solving options, I re-build the form from scratch. I know it sounds like unnecessary work but the form itself may be corrupted and you're just banging your head against a wall.
Starting from scratch doesn't mean you have to do EVERYTHING over. Cut and paste should help you through a lot of it just make sure you test it as you go along to see where the corruption (if that's indeed what it is) lies.
