My data entry and query screens are the same screen. I use QBF Builder.
Menus look like most Windows applications menus. File, Edit, View, Window, Help. I add a Navigation menu after View when a data entry form is active.
Toolbars have Open, Print, Cut, Copy, Paste, Help.
Report form is parameter driven. If you have open a customer screen and click on the Print button on the toolbar, only reports that pertain to the customer will be shown in the Print dialog. If you select File > Print, all reports are available.

I use MESSAGEBOX() for both error messages and alerts.

>(Point no. 2 and 3 need not have a screen shot. It can be a text info.)
