Is the ActiveX Control installed on the system generating the Setup Wizard distribution? VFP selects from the currently registered ActiveX Controls on the system.

An alternative is to include the ActiveX COntrol and supporting files in your distribution directory tree, mark the components as part of the Windows\System directory (under Target Dir) and mark the control itself as an ActiveX item, so that it will get registered by the Setup Wizard.

In general, this is the preferable way to include ActiveX Controls from my POV, since you ensure that any supporting components as well as the control itself are included in your distribution.
