Extremely easy. Most tills have the ability to run off a serial port. Just send the till-specific hexadecimal code out the serial port and, as they say, ka-ching. < g >
There are several ways to read in bar codes. The simplest is to get what is usually called a "wedge". It's a device that sits between your keyboard and computer and when an item is scanned it sends the code (and a carraige return typically) to the computer. This requires some coordination programatically but it's been does literally millions of times by now.
