I follow these standards pretty closely. Basically I have File, Edit, View, Tools, Window, and Help menus. Other menus are added as needed. For example, if the user opens a data entry form, a Data menu appears that gives them navigation, add, save, delete, etc. I also have a Most Recently Used list on the file menu that shows the four most recent data entry forms that were opened.

If you look at some of the data-intensive apps and applets from MS, you'll see Nav commands under Edit, and Add/Delete under file. This is how I group them but, hey, whatever works.
