An example would be surveys that the user could fill-in and attach and send via email. Again, the form must be universally usable, w/o assumption as to if the user has a particular program (e.g. Excel, Outlook) installed. Would custom ActiveX program do this if all use Windows OS?

How 'bout the internet? If all clients have email capabilities, just direct them to your site, place a form there and have the results emailed to you. There are examples everywhere of how to use cgi mail... or if you host your own site on an NT Server, just write the data to a fox table/or similar.
