The SEM is built onto of an object model called SQL Distributed Management Objects or SQL-DMO. DMO takes a snapshot of the state of the server. If you make any kind of changes through the QA while SEM is open and connected, SEM won't recognize the changes until you update the snapshot. Right-click on one of the nodes in the server tree (such as Databases) and select Refresh.
