In my app's project, I specified an icon file which I thought had 16x16 and 32x32 and 48x48 sizes in the same .ico. However, when running the app inside IE, I see the VFP document icon in the browser's title bar and in the Address: section.

1. What did I miss?
2. Does anyone have a Known Working (TM) .ico I can borrow to try to make sure my .ico is not messed up in the first place?
