The second Content-Type line is what bothers me in what you posted. However, my suggestion that [lcHeader=''] could cause that seems illogical now that I see it in print...

Anyway, the extra Content-Type line is what stands out to me as looking out of place in the examples you posted, although I don't know for sure if it is a problem. Just a thought...
