Did you include the Help system runtime files in your distribution? If you use HTML help, you need IE installed on the target system as well as the help system runtime files; I believe that IE 4 or at least the IE 4 runtime environment is needed. This is discussed in Chapter 23 of the Programmer's Guide.

You can create and use WinHelp style help files; VS 6 does not include the WinHelp compiler, but earlier versions did, and you can use them, or a Help compiler like RoboHelp. WinHelp is also discussed in Chapter 23.
