The icon cache has gotten corruped. The easiest solution is to instlal TweakUI on your machine, and when it happens, go to TweakUI's Repair tab and have it Rebuild Icons. TweakUI is a part of the MS Power Toys, which can be downloaded from Microsoft, and are on the WIn98 distribtion disk as well.
