I get this frequently, but I think I caused it myself. FWIW, it happens when I've been browsing around somewhere else and then jump to UT using an IE shortcut. The message is "Blah blah blah... We detected the source to be http://whatever" where "whatever" is the URL of the site I was just looking at. I suspected the problem might be that the UT home page was being called up from the browser's cache, so the login carried the wrong URL. Also I think I had marked the shortcut as "make available offline" (this is IE5). I cleared the offline content from IE and so far the problem has not happened again. It's too soon to tell if this actually corrected it, though.
