Possible things to look at:

(1) Did you include the forms (.SCX) in the application? If not, you need to distribute the .SCX and .SCT files with your application.

(2) Did you hard-code a path reference to the .SCX files (ie DO FORM C:\MyProj\MyForm.SCX) in your application? Hard-coded paths may not be resolved even after you've included the form in your project.

(3) Do you have another set of files in your search path that match the names you specify, but are not valid forms?
