Look under the Visual FoxPro directory. You will find a sample directory which an application example which is known as tastrade.

I tried this using the list box and can't get it to accept the 'ENTER' after user puts in his name.
If both data entry need something to be entered by the user, you may want to use a textbox field for both of them. Just use the ControlSource property to map the field to the appropriate field in the table. This should give you a good start. Of course, you may want to define your form to be private, to add the table in the DE and to plug a buffering for that table.
