This is a small app...one form/menu, no application object/form manager. After further discussion (and a reality check from Bill Caton), we realized we were trying to over-OOPify the fool thing.

The Form Manage needs to be referenced by a global (note I did not say PUBLIC) memvar that the menu can see.
