Thanks Jeff and Rob for the help. We were able to find Article ID: Q157172 - How to Create Custom Forms in Windows NT 4.0.

The FPW26 report was not actually mine but by another department. They have
a client running the app under WinNT 4.0 and we didn't know how to add a custom form size in that OS.

>Arriyel, the easy way to fix the problem is to go to the printer itself and change the paper size. Most dot matrix printers allow changing the paper size at the printer. That also means that the printer will have to be changed back after the job is done to the original paper size if other things are printed on the printer.
