It's true, you can't create a custom paper size from a workstation. It must be defined at the SERVER as follows:

On the NT Server open the printer folder
Highlight the cage card printer
Select the file menu
Select print server property
Select the property window's Form tab
Check new form checkbox (midway down)
This enables desc, name, hieght, width, and margins.

We did have some printer alignment problems using an Okidata print driver and FP2.6. In order to get a 3" by 5" we had to set it to 3.63".
