I realize I may have mis-understood your question. If you're looking for a reference for HTML Help itself, try http://msdn.microsoft.com/workshop/author/htmlhelp/ as a starting place. There is an HTML Help API reference at http://msdn.microsoft.com/workshop/author/htmlhelp/hhapi.asp.

Also there is a book from MS Press entitled "Official Microsoft HMTL Help Authoring Kit" by Steve Wexler (see http://mspress.microsoft.com/books/1408.htm). It's two years old and somewhat out of date now, but not a bad place to start.
