First of all, does any part of the form load, at all? Forms are stored in data tables with .SCX/.SCT extensions, if that table has been corrupted, your form will not load.
Another possibility is that the form loads, but one of the controls is bound to a data table (.DBF). If that table is corrupted, you may also get this message.
In either case, if you don't have a backup, you'll need to rebuild or recover the file. You can try a file recovery tool right here from the UT (click on the Files button in the top left, then the "Databases, tables..." icon, then download file #
9125 - "CM Table Repair Utility". If you're lucky, the table header may be corrupted, which can often be fixed without data loss. However, if you lose data in the repair process, the form file (.SCX/.SCT) will likely be unusable, and you'll have to recreate it from scratch.
Back up any form files or data tables before attempting to repair them.
Finally, set up a good backup system.
