I think what Evan said is correct. If you look at the SMTP header, it shows each SMTP stop along the route. I guess if you connected to an SMTP server far enough away, the IP trace wouldn't point to you.
Also, you have to find an SMTP server out there that's willing to take your e-mail and forward it. Almost all require some type of authentication because they don't want SPAMmers using their resources.
