Have you checked out hentzenwerke.com? Whil sells a number of books that touch upon this subject area. In particular, Jim Booth's book is excellent in showing how to build applications from the ground up and Peterson/Paddock's book on Enterprise issues, including client/server is good as well.

Building your subclasses, include files; normalizing your data and inserting business rules, etc. An application that incorporated commonly needed features such as data maintenance forms, user access, etc. And then continued to build it up, say to client-server &/or make it internet enabled. Is there any book out there that does this? I am not capapble (yet!) to write such a book, I am looking for one and suggesting it.
