For me, the issue revolves around the size of the bar code. The bigger the bar code the more readable.
Also, most scanners are configurable as to how they interpret the bar code.. i.e. they can be set up to append any kind of data at the end of the bar code. This is a better solution than adding a "+" as depending on the scanning equipment at the other end, it could be interpreted as a "+".
If you are not using a wand, but a CCD or Laser device, you can often get away with a "poorer" quality bar code.
To make it readable at the other end, double the size of the minimum readable bar code at your end.
This should work reliably. (Have done this with Intermec equipment, successfully.)
