I recently asked the same question, so two other alternative are:
Use Dragan's NewForm form from the UT files section (I modified it a little (add few simple improvements) and then sent the new version to Dragan , haven't checked, if Dragan updated it or not).
The second alternative - use Ilie Tomac form wizard. He sent it to me in personal mail, I like it and will ask him to put this wizard here in UT.
