I will try with REINDEX.

I test the table with this:

I save the form in question like a form "FormError". In this form I add a button. The event click() of the button it's a curl to repeat all the action of the user.

With this test the "FormError" scan all the record of the table and I don't get the error. I have 2 Pc run this "FormError" and I don't get the Error.
