Beat the h*ll out of me ;-) Most of the C5's that I have seen (with SP3 installed) are generally due to what we euphemistically call BDPE's (Brain Dead programmer Errors) ;-) I have personally not seen one unless I have some dangling object references lying around.

When it comes to reports, I am no help at all because I simply don't do them. As a matter of fact, we really needed Rick bigtime to do the chapters on reports because the only tip that Andy and I could come up with was 'Hire a contractor to do them for you'.
