I use Erwin from Platinum Technologies (now Computer Associates) (http://www.cai.com/products/platinum/appdev/erwin_ps.htm). I use an older version (3.5.2) and I think they have since integrated more closely with some of their other utilities. It can Reverse Engineer your SQL database and gie you a detailed graphical look at your design.
