The standard answer would be make sure you have the service pack3 installed.
I have very few problems when the SP3 has been installed.
I have also had luck with making a dummy installer which only installs the VFP dll's.
And so taht it registers the vfp6r.dll
i.e. regsvr32 vfp6r.dll you can of course do this in the installer.
I don't recall anything specific with the do form command.
