After playing with this some more, we've found that it is definately the AOHell client. It displays a non-standard login dialog. We can dial in through AOHell, minimize it, launch IE, and then enter the IP address of the web site. This works fine. So much for AOHell and their "We fully support IE 5.0" and "We embrace the open standards of the Internet". We're not doing anything other than standard HTML. As I said, AOHell also chokes on Basic Authentication.
