2 ways:

1) Go in Tools|Options..., select the Forms tab, check Form in the "Template classes" section, select your form class and click OK. The next form you will create will be based on this form class

2) In the command window, type CREATE FORM MyForm AS MyFormClass FROM MyClassLib
