In all probability, you are not distributing the needed compiled HTML help drivers, and the target machine has never had the necessary updates applied from IE4 or IE5. You need to download the patched HTML Help components and distribute them as a part of a Setup Wizard generated install (you should apply VS SP3, which includes the necessary HTML Help update as well as lots of bug fixes.)
