What does it mean and how can I debug it?

Just started happening tonight. Never seen it before. Haven't been playing with the menus and have no idea what caused it. It causes an immediate exit from vfp.
Problem occurs when a form I'm working on crashes and I issue the normal closedown commands to clear the forms and other stuff from the environment.

All hints welcome...
