The ACH record formats are spelled out in the ACH Operating Rules (Corporate Edition) published by National Automated Clearing House Association (NACHA). You can contact them at 607 Herndon Parkway, Suite 200, Herndon, VA 20170 tel. (703)742-9190. I don't know of any pre-existing templates, but you might also try the NACHA web site at www.nacha.org.
