Then I added the table back to the database and re-entered the index info, and responded yes to the 'make changes permanent' message. It got to 99% complete and then hangs. The table is only 40Meg, and I have 64Meg of RAM and 6Gig of disk space.

Try SELECTing all records from the table into a local copy... then ZAP the original and APPEND FROM the backup.
