Use a report variable that will sum the field and has the Initial Value = miscellaneous.

Or use a variable that sums the field and you print variable+miscellaneous (as the expr of the printed field).

FPW seems to sum miscellaneous charges as many times as the number of details if I use subtotal+miscellaneous as the variable and select calculate sum.
