>>I paid a flat annual fee for a $1,000,000 E&O policy, not sure exactly how the cost was based, there was 4 page questionaire I filled out along with submitting financial statements & marketing info examples. Ended up costing me just over $1500.

Hi Cathy,
I actually went thru a "broker" instead of an "agent". The broker took my application and other documents and sent it out to all these other agencies/companies as an RFP. He sent out 15 RFP's, and got back 6 bids. Of the 6 he recommended only 2, with the one being substantially better coverage (basically no riders whatsoever)... so much better it was pretty much a no-brainer decision which one to accept. Now the name of that particular insurance carrier currently eludes me, and my financial advisor has my certificates & whatnot at his office at the moment. As soon as I get my insurance docs back from him I'll email you the name of the carrier.
