I still have a fair number of clients who use 640x480, mostly out of ignorance (don't realize they can change the property). I've taken the approach that if they want me to continue to upgrade something, they have to use 800x600 (except for DOS systems, obviously). And for any NEW projects, I tell 'em up front - 800x600. Most of us can't afford to be developing systems at 640x480 (especially if we re-use certain classes). I tell them nicely, but I tell them firmly. No exceptions (I guess for the right money, I'd make an exception - at least 2-3x what I would do it for otherwise).

For most new work I find that's acceptable to clients, and it certainly gives me additional freedom to design more useable forms. Some of my clients still have older workstations using 640x480, though, so I've just got to remember that when doing things for them.
