I would recommend using Image . It came with VFP5 but I don't think it comes with VFP6. Not sure about that. Look around.

I only do the "EGA/VGA 16-Color 32x32" and "Small Icon 16-Color 16x16". Make sure you attach it to the project in Project Info. You'll still have to set _screen.icon when you start your app.

One last little thing... I think (again not sure here) that at least one pixel has to be transparent for it to show up in the start menu.
