You have to create a formular with the correct size or choose one of the already existing formulars.

Creating a new one is simple. Go to the printers folder, RightClick and choose the print server entry. Choose an existing one that aproximately fits to your needs. Check the "create new" checkbox give it a new name and save it after you altered the settings. As long it's size is printable by your printer you can choose it now in the printer dialog.
