Almost anything - there's no standardized format. It's used in any number of databases and files formats for things like COBOL.

THere's no standard answer - you need to know what created it. There are third-party products like Data Junction that may be helpful (they have translators for lots of different formats built in) but at some level, you'll have to identify the source, and most likely the internal structure of the file.
