Tim and Ko are both right. You first have to start with a Top-level form. Next you need to change a couple settings when you generate your menu.

Before generating it, go to View - General Options and check the Top-level Form option. This will add a comment block to the MPR file giving you directions of how to implement it as part of the Top-level form. Basically, you have to run the MPR from the Init and pass the form as an object reference to the MPR code.
