A little over a year ago, our team did some preliminary work on integrating these two things into a POS application. Unfortunately lack of funding caused the caller ID requirement to slip but Zeus Phonstuff's product Whozz Calling looked very promising. You can find them at www.callerid.com.

As for receipt printers, we used an Ithaca model and (as Mark suggested) it included both a DLL and ActiveX library to use to access it. You can find more information from www.mscashdrawer.com (they resell a lot of POS equipment).
