In step #2 I select "Active X" controls but the MSCOMM control is not in the list. I review of the dependency file does not show the MSCOMM32.OCX file anywhere either. How do I make sure that the communications control is included with my application so it will be installed when setup.exe is run?

Copy your mscomm.ocx file into your distribution directory. And you will see the ocx file in step 6. Look at step 5 for program group.
