"Manually" should mean really typing "do form blabla" in the command window, or building the app and running it from there, to differ from rightclicking and picking "run form" from the context menu (where it is supposed to ask you to save first - and that save seems to be incomplete at times you described).

What I suppose is happening is that your instance of the form is doing a oFormCurrentlyInDesign.SaveAs([scx name here]), instead of oFormCurrentlyInDesign.SaveAs([scx name here], oFormCurrentlyInDesign.DataEnvironment). That's the easiest way to lose a DE programmatically, and probably (just my wild guess) what accidentally happens.
