I once worked for an insurance company in Toronto where the Auto department was mmm/dd/yy and the property department was dd/mm/yy. The reason was that both companies had been purchased and their systems left in place. Of course all the agents and customers thought we were idiots, no one ever paid anything on time !! And overdue letters were common as snowflakes!
Whenever possible, I use default date so the user gets his own setting ( and I wrote a warning to tell if they are at variance with the application default, and I try to use MMM/DD/YYYY on reports
