I did see your post elsewhere. FWIW, it would never occur to me to look under "Voting machine ballots" to see the form for this kind of voting. To me, a voting machine is a device that records the votes and you don't have to insert anything or put anything in a ballot box. These folks are using _paper_ ballots.
