A TX form from the departmwnt of commerce. An attorney will do it for you. You will need to assert ownership and attach the first 25 pages and last 25 pages of source code.
And how do I get them?
Firms like Gunn Lee and Miller specialize in them. You can call your district congressmans office to get information (thats free). A book - "Legal Care For Your Software", will make good reading.
