Good job on your funky form! I have no idea how you accomplished it, but it's pretty cool.
Just thought I'd point out that you might want to make a correction to your CusFormTransformer.doc, SetFormTransparent example:
"oformtrans.SetFormElliptic(THIS)" should be "oformtrans.SetFormTransparent(THIS)".
