I'm successfully using the Amyuni PDF driver. See www.amyuni.com for more information about it. The latest version allows concatenating files. This is great for developers, though the license can be $850-1150 to incorporate it royalty-free into your app.

I don't know if Adobe Acrobat can concatenate (I assume it could) -- it's per seat cost is ~$250ish.

There are plenty of examples for both packages in the files section; I'm sure others will jump in and give more specifics.
